HEALTH ADVISORY: Measles Exposures in New York State

in Clinical & Quality

We have received the attached notice of measles exposures in Wyoming County, who has reported 5 cases of measles in members of the Mennonite community. The index case has not travelled outside of Wyoming County and the source of measles for the index case is unknown. This means that measles may be circulating in Wyoming County. Health care providers need to maintain a high index of suspicion for measles in anyone who presents with fever and rash. In addition, please be certain to ask about possible measles exposures for those who present with fever, cough, conjunctivitis and coryza. Health care providers should increase their index of suspicion for measles in clinically compatible cases. In NYS, confirmed or suspected cases of communicable diseases are required to be reported no later than 24 hours from the time the case is first seen by the physician, however, the LHD should be notified of any suspect case immediately. Reports should be made at the time of initial clinical suspicion.

DOH: HERDS Critical Asset Survey - shutdown coming on 12/19/18

This message is to inform all Health Care Facilities (HCFs) - hospitals, nursing homes (NHs) and adult care facilities (ACFs) - that the Critical Asset Survey in HERDS will be temporarily shut down - starting today (Wednesday December 19, 2018) and continuing through January 1, 2019. 

During this shutdown, the three current forms of the Critical Asset Survey (Baseline/Surge/Resilience) will be merged into one, combined form going forward. All data that was previously submitted in both the Surge and Resilience forms will be migrated to the new, combined form and therefore will not need to be re-entered. 

The Critical Asset Survey is being reactivated on January 1, 2019 - in advance of the rollout of the Facility Evacuation Planning Application (FEPA) in New York City. A Dear Administrator Letter (DAL) regarding the Facility Evacuation Planning Application (previously known as the Facility Profile Application) will be forthcoming to HCFs in NYC. 

The Critical Asset Survey review and update period will run from January 1 - March 31, 2019. All Health Care Facilities are required to review their previously submitted data, update any that has changed and (re)Submit the Critical Asset Survey data during this time period.

Influenza Declared Prevalent in NYS for 2017-18 Season

The New York State Department of Health Commissioner Dr. Howard Zucker declares influenza to be prevalent in New York State for the 2017-18 influenza season, as of the date of this announcement. In accordance with Section 2.59 of the New York State Sanitary Code (10 NYCRR § 2.59), all healthcare and residential facilities and agencies regulated pursuant to Article 28, 36, or 40 of the Public Health Law, shall ensure that all personnel, as defined in the regulation, not vaccinated against influenza for the current influenza season wear a surgical or procedure mask while in areas where patients or residents are typically present. This declaration shall remain in effect until the Commissioner declares influenza no longer prevalent in New York State. See attached for more information.

NYSDOH is requesting a review (and update) of Critical Asset Survey data by all Hospitals, Nursing Homes (NHs) and Adult Care Facilities (ACFs) statewide.

ALL facilities are directed to review their Critical Asset Survey data to ensure that it contains accurate and UTD information - AND to answer any newly added questions. This includes the Baseline form PLUS the two supplemental forms (Surge and Resilience) included in this HERDS Activity.

At the bottom of each form, an Attestation has been added. This attestation MUST be completed on each of the three forms in order to Submit them and provide DOH with an updated Completion Date for all forms.

The Attestation will also serve as confirmation that all data was reviewed and updated if/when necessary - even on a form where no changes are made otherwise.

For this update period, the following changes have been made to the survey forms: 

  1. the Baseline form for all facility types has been updated to now include a section on Electronic Health Records (EHRs).
  2. the Baseline form for Hospitals/ACFs now contains a section on Collective Bargaining Agreements. 

This update should be completed and ALL THREE forms Submitted by Friday October 6, 2017.

On September 8, 2014 the Drug Enforcement Administration (DEA) released their final rule regarding the disposal of controlled substances which takes effect October 9, 2014.  This rule governs the secure disposal of controlled substances by registrants and ultimate users. These regulations will implement the Secure and Responsible Drug Disposal Act of 2010 by expanding the options available to collect controlled substances from ultimate users for the purpose of disposal, including: take-back events, mail-back programs, and collection receptacle locations.  Significant changes are made in this final rule which the DEA states are to help ensure that long term care centers have adequate disposal options...
